The Family Portfolio
Family memberships may be purchased.

• Eligible Family Members include spouses, children and parents.
  
• One (1) person, the Prime Member, will be responsible for the payment of all dues. The failure of the Prime member to pay any portion of the dues will jeopardize the entire Family Membership.
  
• Family Members may select any level of membership. However, Family Memberships may not be purchased at a higher level than the one selected by the Prime Member.
  
• The initiation fee for the Prime Member will be at full price. Each additional Family Member’s initiation fee will be discounted 50% of the full rate for their selected level.
  
• Monthly dues will be at full rate for all Family Members.

Increases in fees may be made at any time to any membership, but will not be applied until a Member’s next anniversary. We reserve the right to change specific procedures at any time and will provide an appropriate alternative. Treatments must be used per the portfolio schedule and cannot be accrued or transferred. Each member may have up to 2 guest visits per year.
